I am unable to enter daily appointments into the calendar

Each time I click on a date to enter an appointment, it says that the form is
unavailable and to contact the administrator. I'm not sure if the appointment
form was accidentally deleted, but we have been unable to locate it. If
anyone is able to help I would greatly appreciate it! Thank you.
