I can't create a folder in my exchange mailbox.

This problem has just shown up recently. I can delete and move folders
around so I do not feel permissions are the issue. I am able to create a
folder / sub folder through the OWA with my same user profile. There is not
sufficent information to trace the cause of my thorn.... Any direction would
be greatly appreciated.

Try creating a new mail profile and testing.

Just to clarify, I will create a new exhange mail account and set this
particular outlook 2003 client to use the new profile and see if the problem
persist.

I will also try using a different outlook 2003 client (different machine)
with the same credentials as the one with problems and see if it follows.

There used to be multiple mail "profiles" in the outlook client. But I do
not see them anywhere, at least not in the client the way we have it deployed
for exchange.

If I have missed the point let me know. Thanks!

Control panel, mail - display profiles?

No, I think you're on the right track - just try another OL profile with the
existing mailbox first.
