I cant find form page wizard

I am trying to create a form using the form wizard in Front Page 2003, and
cant find the wizard, help?

Make sure that you have site / web open and not just a page. Also check your authoring settings
under Tools.

there's a little page icon on the FrontPage toolbar, next to the folder
icon, click the little arrow next to it, then select "page", it'll throw up
a dialog with some page templates, one of them is the form page wizard.
