Are you talking about filling it out with Microsoft Access data? or in
Word or in some other program?

I have put it into word. I don't know Access. The options from my scanner do
not include Access?

I ask because you posted this question in the newsgroup
microsoft.public.access, which supports the Access database program.

If you're working in Word, please repost your question in a newsgroup
which supports Word. I'm not certain how you got to this newsgroup
(from the Word online help perhaps?) but there should be a list of
programs, or a dropdown box defaulting to Access; use it to find an
appropriate newsgroup.
