i need to use a template on and off the computer.

I've got an excellent template for substance abuse assessment on-line. The
problem is I not only want to be able to have it filled out on the paper, i'd
like to be able to answer the questions right on the 'puter and then print it
out. But i can't Put X's in the boxes or keep the form and appearence of the
template.
