I'm sending out an e-mail invite and want them to reply in a form

I have a holiday invite to a pot-luck. I want people to:
1) RSVP
2) indicate on a form using a check box what they're going to bring -
potatoes, salad, appetizers, etc.

Outlook doesn't support forms. You could use voting but it only works with
outlook, not other mail clients.

I'd just ask them to reply with their choice.
