Importing form fields in Acces

I have made a nice outlook form but want tot import te fields in MS Acces
database, i've searched the internet for such a solution but can't find
anything about it.
Does anybody know the solution?

Outlook doesn't support exporting custom fields. You'd have to write custom code or use a third-party application. See http://www.outlookcode.com/d/customimport.htm .
