I use included content to form pages - which all work fine except fo
one issue. When I insert a form page and publish the form shows u
twice. There is no code that I can see that makes this happen. I'v
tried inserting it into a separate table, but same problem. Sometime
it shows up 3 and 4 times. Any ideas

There are some bugs in FP2002 & in the FP2002 SE related to shared borders & include pages
so make sure you (and your host) see & apply all appropriate patches from:
