Index displays page number, but no corresponding text

I have an index that in some places displays the page number, but doe
not have the associated text. This seems to occcur when text in th
document that had a hidden index marker is deleted. As an experiment
I deleted all the "XE" markers (Find and Replace ^d XE) and the
updated the index. All it shows is page numbers. So my question is
how do I get rid of these displayed page numbers from index marker
that have been deleted
