Information Form creation

Hello pros...

A few years ago I "installed" information forms at the bottom of a few pages
on my website that remained "under construction!" Now I would like to
complete the installation. What I mean by that is I would like to have the
information submitted to be sent to me at one of my email boxes. But, for
some reason I can NOT determine how to do that, nor exactly how I created the
information forms in the first place -- years have taken their toll,
apparently. I'm not a FrontPage expert by any stretch of the imagination,
although I've built several sites, for our offices, which aren't too bad.
The information form creation process simply escapes me after all this time.

Here's what the form appears like within FrontPage:


Information Request Form
Select the items that apply, and then let us know how to contact you.

Send service literature
Send company literature
Have a salesperson contact me


Name
Title
Company
Address
E-mail
Phone

Submit Request (tab) Reset Form (tab)

If the submission of the form can be done via email, then how do I go about
providing that info? With the little I can remember, I tried to right click
on the Submit Request tab, then Send To I entered the email address, but that
was rejected, "This form cannot be configured to send results via e-mail."
Most likely I was going down the wrong path?!

Most likely I will add a text box for users to enter a message? I don't
know why I didn't do that at the time I "created" as much as I did? Your
thoughts on this issue would be useful, too...

Your help would be ever so greatly appreciated.
