Information is repeating itself halfway through the document

We are writing a multi-page document including graphs and charts. When
scrolling through the document about halfway through one page contains a
large chart. The chart is missing from the page and then the document
repeats itself from page 1. As a result the last third of the document is
missing.

Sounds like you may have gotten a bunch of text inside the bookmark for a
REF field.
