Insert a Radio Button and Text Field

Looking for a way to insert a radio button and text field in Outlook - or
something similar - to put it in an outgoing email. Milly says in Outlook to
click on Tools - Forms - Design a form. I don't have anything under Tools
that says Forms or Design a form. Also there's nothing anywhere on the top
menu that says that, nor is there anything in Help that lists this. Is there
another version of Outlook that maybe I don't have??? Thanks for your help.
