As someone commented in a Nov 2005 post, InfoPath by default does not
allow images in headers. Here's a workaround:
1. Tools > Options > General > Check "Print background colors and
background pictures" on
2. File > Page Setup > Margins > Top = .25" (or whatever works for
your printer)
3. View > Header and Footer > General > Check "Use a background
picture" on, browse for the picture, leave Tile options unchecked,
Position = Upper Left
4. Place your top table/body text at least .25" below your header
