Insert Object Adobe PDF File, Icon is empty box

We have a document where we inserted an file into the the
document. The file is a PDF file. On some machines the
icon that displays is the adobe system, on some machines
it is just blank with just the label appearing. If you
highlight the blank icon you can see the black box around
it. If you double click Acrobat brings up the file for
viewing fine. Any ideas why the icon is blank? Thank you.

We had the same problem here. The solution is to click on the "display
as icon" box BEFORE selecting the file.
