Insert text based on checkbox

Is it possible to have a field which displays an informatin based on what
checkbox is selected on a form?

Lets say I have an address field. If the user clicks on checkbox 1 address 1
is displayed. If the user clicks on checkbox 2 address 2 is displayed. Etc.
