Inserting asked information into a document

When a documents opens I want to ask a question to which the user
replies then insert this reply into the document?

Mark,

In your document template insert an ASK field. In each place you want the
data to appear insert a REF field to the ASK field bookmark name.

Example: { ASK "Name" "What is your name?" }

My name is { REF Name }

{ } are entered with CTRL+F9.
