If the form you are creating is a Word 2003 document then it's really not
the best tool for what you want to accomplish.
You can insert a Continuous Section Break in the document, say at the bottom
so the file can be attached, and then when you protect the document, specify
that section not be included in the protection. Then the end user can use
Insert/Object, use the Create From File option and they can using the
"Display as Icon" option.
I'm still not sure if something like this is what you are looking for....
