Inserting form fields into a text box

Hopefully someone can help me with this problem.

I'm attempting to create a template of a document that my
coworkers and I have been having to complete by hand. To
create the document, which has most of the text in boxes
(such as name, date of birth, things like that) I drew
text boxes and then typed the question above in the box.
My intent was to insert a form field just under the text,
but for some reason the form field button on the form tool
bar is not usable when I have the cursor in the text box.
Anyone know a way to get around this?

Any help would be greatly appreciated.

Use table(s) instead of textboxes.
