Is it possible to add note to time entry rather than task?

We would like to keep track of what was done each time a
person works on a task and associate it with the time
entry.

For example, a timesheet entry might look like this

Task
Resource
Date
# hrs worked
Description of what was done

Can this be done?

Not really... This would get unwieldy rather quickly. Better way is to
have people write documentation and store it in the Documents store for
the project. You can then associate the document with the task.
