Is there a good template for a post meeting de-briefing?

I am trying to find a good template or reporting style for an assignment that
I have been given from my boss. I have to do a follow up to a Team meeting
that he headed.

You're asking in the wrong place. This newsgroup is for Microsoft
Access database design - not for general computer questions. For that
matter, I suspect that the CONTENT of the followup report is a heck of
a lot more important to your boss than the Word Style (which you might
be able to find on a Word newsgroup). Maybe... ask your boss what s/he
wants?
