Issue - Access Denied

I have a Team Lead person that can't actually add an issue. The project is
fine and has a sharepoint site. When she goes in and looks at issues, she
can do everything in there until she hits the save and close button. Then
she gets the messages that I have pasted at the bottom. She is a Team Lead
with permissions to "create new task or assignment" and "view risks, issues
and documents". She has "new task assignment" and "new project task"
permission also. She has no "deny" anywhere in the group or her personal
permissions.
Here is the error she gets:
Access denied. You do not have permission to perform this action or access
this resource.

Access requests are not enabled.

There are two different Issue, Risk, and Documents permissions in the
security template. The first is under the "Collaboration" security heading
and there is a different check-box for each of the permissions. The
description for this Issue permission is: "View Issues is a global permission
that allows a user to view the Issues center in Project Web Access or from
the Collaborate menu in Project Professional. Grant this permission to any
category of users who need to add or update project-related issues, or link
to tasks or documents in any project-related site the user has permission to
view."

The other permission is a single check-box for all three is under the
"Views" security heading. The description for this Risks, Issues, and
Documents permission is: "View Risks, Issues, and Documents is a category
permission that allows users to view Windows SharePoint Services Risks,
Issues, and Documents areas in Project Web Access and Project Professional.
Grant this permission to any user of Project Professional who needs to select
Documents, Issues, or Risks on the Collaborate menu or any user of Project
Web Access who needs to access the Documents, Issues, and Risks top-level
navigation links."

Make sure both are checked.
