Junk E-Mail not blocking

Junk e-mail option is set as "safe list only", Safe
Senders tab "Also trust e-mail from my Contacts"
and "Automatically add people I e-mail to Safe Senders
list" are both set.

Some senders on the safe list are sent to the Junk email
folders, some senders not in my Contacts and that are Junk
show in the InBox.

Suggestions
