Label Mail Merge

How do I get label mail merge to put different addresses on the merged
document instead of all labels having the same address?

The document is merging but I am getting one sheet of labels (6labels per
sheet) per name in the names documents.

In my own limited experience, I just went through that. Each label after the
first one (top-left) must have as its first field "next record".
