Lines on a form so others can edit and type info ON the line

I am trying to create a form. There will only be a couple of lines that
require person editing the form to type ON the form without the line going
away. Would like to also have a few lines side by side, i.e. see below

Manager Date

Would like others to type ON the line without line going away.

Use table cells to create such a form. See the article at

article also has links to articles explaining how to create documents with
forms protection, but from your description I don't think that's what you
need.)
