link an exisiting table to a new table via a form

I have customer complaints coming in everyday, the main base table is called
'clients' I want to build a new table 'CustomerIssue' and linking them to the
'Clients' table via the use of a form. On the form, I want to enter the
reference, which is held in the clients table to be able to:
1. link the records in a relationship
2. set a column in the Clients table to say that we have an Issue.
On the form fill in the details of the Issue which will go into the
CustomerIssue table.

Hope someone can come up with a solution and thanks in advance
