When I create a form to submit, is there ANY WAY to not have every item show on my end when they hit submit and it goes to my email? For example, I have created a supply requisition for my company's employees, and they may need 1 ream of paper, yet the form is showing all 50 items as a zero for not needed. I just want it to submit the ONE item that they have checked, not the entire list

Got it? I think I confused myself! Help!
