Locking only part of a sentence

Please Help Granma,

I have a template that I use to type info into to send out to people in PDF
form. How do I freeze the info that is already in the form ex..Name, Address,
and Phone number are all listed on the right side of the form and after each
I type in new information for a number of people. All the info is different
for each person except Name, Address & Phone etc. . I want the left side info
not to move. I hope I am expressing clearly.
