locking Templates/Forms

I am creating a form which will be completed on the computer - the form is a template document.
I would like to lock the form so that although people can enter data into the relevant fields they can not alter the format, colum headings etc..

Any help is appreciated!

What you are talking about - looking for - is probably what Word calls an
"online form." Check this in help. You could also take a look at Dian
