looking for a template to create insert for a DVD 7.5 x 5.25 case

Help, I need to find a template to create inserts for DVD cases that measure
7.5 x 5.25. Looking for front spine and back on one page. Can anyone help?

Mary I must be a bit green at this but I can only find templates for the
"jewel boxes" not for the bigger "book-type case" that measures 7.5" x 5.25".
Any suggestions?
