To create your macro, click on Macros, New,
Click next to Action.
Scroll down to OpenForm
In the Action Arguments box at the bottom of the page
Click next to Form Name and choose your LSJChocolates form
Save your macro by pressing the Save button on the toolbar and give it the
name you want.

To create a new toolbar, in your main database view, right click on a
toolbar, choose Customize.
Click on the Toolbars tab and press the New button.

When you have created your new toolbar, right click on it, go to Customize
and Commands. Scroll down to All Macros. You will see your macro there. Drag
it onto the toolbar.
To change the icon, again click Customize,
right click on your new button, choose Change Button Image
You will see your runner in the list.
I'm not too clear what you mean by the last bit of your message but you can
add a command button to your form in design view and the wizard will let you
open a form with it.
