Mail Merge Toolbar

I need to teach students using the mail merge toolbar and am stuck in how you
go about creating the data source icon on the toolbar. Can anyone help?

The data source icon is on the toolbar by default. See

If not, reset the toolbar (little arrow right end of toolbar > Add or remove
buttons > mailmerge)
