Mail Merge with conditional formating

How do I create a destination merge document that will automatically create
check boxes based on the data merged in?

Save the checkbox and uncheckboxed characters from the wingdings (or other
symbol type) font formatted at the requires sizes as autotext entries. Then
use a conditional field to place them (you could if preferred simply insert
the characters directly into the conditional field in place of the autotext
fields) eg

{IF {Mergefield fieldname} = "condition" "{Autotext checked}" "{Autotext
unchecked}"}
