Major problem

I don't know what to do - I have followed the instructions and my web pages are not working correctly. If I could get someone to take a look at my web pages and let me know why they are not working that would be great. http://216.39.194.182,My UBS project/Main web page.htm
when you go to this main page Financial Calculators, Contact Information and UBS Financial Services Web Site work perfectly. The other three send you to the correct page when clicked, but then when you choose an option (take Schedule a FREE Consultation) like yes, I right clicked on Yes and entered a hyperlink under properties to link to another page, but instead of going to that page I get a pop-up window asking for a user name and password. I am only having this problem on these three pages and they all have radio buttons. What have I done wrong? These pages also have a weird statement - "FrontPage save results component" - that's not supposed to be there - Where does that come from? Thanks in advance for help!

First get rid of all spaces in folder and file names.
Second, do not place any pages in the _private folder.

The error message is generate on any page that uses the Form Handler and you
have not published it to server that has the FP extensions or you used FTP
to upload the content.

Well for starters I got a 404 when I clicked the link.

Second, replace the spaces in all of your file names with
underscores _ most browsers do not handle spaces in file names.
The spaces are what cause %20 to show.
