Making a custom appointment form

Hi all... I have never done this before so do not even know where to start.
I need to make a custom appointment form with time, BC No, Name, Phone
Number, Site Address, Special Tnstructions and Inspection Type.
I then need this information to display when the calender is open. Also,
they need to be able to print each day in a table form showing times in half
hour slots and any inspections they have for that day.
Where do I start? How do I go about it?
