Mathtype Toolbar

Assuming you have MathType installed, you may be able to select the toolbar
from the toolbar list, or it should appear when you insert a MathType
object. I'm guessing, though, based on the behavior of Equation Editor,
since I have no experience with MathType.

How do I get the toolbar for Mathtype to appear in Word
on Office 2003

If you mean "how can I get the MathType symbol and template
palettes to remain on Word's toolbar?", the answer is you can't.
When you install MathType, it should install the "MathType
toolbar" and MathType menu onto Word's toolbars and menu bar,
respectively. The MathType toolbar contains icons that let you
open MathType to insert equations, among other things. You can
identify the MathType toolbar by the presence of four blue Greek
capital letter Sigma characters. More information about the
MathType toolbar in the MathType manual. If you don't have a
printed copy, you can download a PDF copy from our web site.
