Is there any way to remove or change the "Microsoft
Access - [FormName]" and Access icon in the banner of a
form window?

You may set the
Caption Value
of the Form

You can add your own icon in the Startup option (under Tools). Changing the
caption property of the form will replace whatever the default is with what
you type in there.

I don't know about the banner - but the FormName is only displayed if there
is no "Caption" set for the Form Properties.
