If its asking for your credit card information, and other personal
information, it has absolutely nothing to do with Microsoft or one of their
products.

It most probably is a virus. Do NOT give the form any personal information,
including bank account numbers, credit card numbers, email usernames,
passwords, etc.

Run an up-to-date virus scanner IMMEDIATELY!!!
