missing formatting palette

My formatting palette disapeared! When I hide/unhide it from the menu I
can see it zooming across the screen.... right off of the screen!

Has this happened to anyone else? How do I get it back? I can't find
anywhere to reset palette locations or anything of that nature. I am at
a loss.
