missing menu commands in just one document

hi,
for msword 2003 on xp.

for one document, not on others or blank docs, the page numbers
command in insert, and the header and footer command on view are
missing.

if i drag them in using tools|customize then the other docs have them
twice.

i have run repair. no diff.

suggestions?
