Missing or broken reference to the file 'DA0350.DLL' version 5.0

I am trying to use the template from the link listed below in Microsoft
Access 2003 running on a Windows 2000 Pro/SP4 desktop.

I keep receiving the following error when launching the template:

"Your Microsoft Access database or project contains a missing or broken
reference to the file 'DA0350.DLL' version 5.0.

Any ideas, plz?

I do the the latest Jet Database loaded as well and all updates from windows
update

Access 2003 shouldn't be using a reference to DAO350.dll: that's DAO 3.5,
which is Access 97 only. Try changing the reference to DAO 3.6 (Tools |
References when you have the VB Editor open)
