Missing Third Address Line

I am doing a Mail Merge to generate Avery 5160 labels. I map my fields,
including Address 2 and Address 3, and run the merge. Everything looks good
EXCEPT that the third address field is missing.

Address 2 shows up fine, but Address 3 is consistently missing.

Any ideas?

Don't bother with mapping fields, just insert them in the configuration that
you want.
