Most 3 recent dates in separate columns

Please assist.
I need a count of patient visits in the most 3 recent dates for each clinic
with the dates in separate columns.
I attempted to use a crosstab but I could not use top value feature in query
properties.

query fields:
visit date
clinic
patient#

Create a query that returns just the top 3 dates per client. You can then
use this query as the source for your crosstab.
