You most likely will have to do a lot of customizing to make any template
you find really work for your organization. I present some ideas below to
get started.
You need to determine what 'entities' you are working with, you have a good
start below, but there are more.
You most likely have some 'data' (maybe 3x5 cards) to work with review that
carefully for data elements of interest.

The above ideas are just a start, but you can see there is a lot of planning
before you sit down to build the database and it's related user interface.
