MS WORD Files reverting to documents partially previously saved

Okay, at first I thought I was crazy, but now after multiple occurences I am
sure that MS word is either not saving my revised documents at all or is
saving my documents as i direct, but reverting back to a formerly saved or
autorecovered version. This has happened twice now and is causing my
academics to suffer. I save a whole assignment. When I go back to the
document a large protion of the supplemented work and revisions are missing
please help me. I save the documents to a flash drive and to the hard drive
to try to prevent these occurences. Could this somhow cause the ocurences. I
am desperate. please help me.

Are you positively not using "Track Changes" in this document (but if
you do: are you sure that you are showing the right "state" of the
document)?
