Multi level grouping

Creating report to summarize medical payments by insurance and patient visits.

Fields include [Date From] (date of visit), [INS1] (insurance name), [Chart
Number], (patient ID)

Would like the report to summarize
per year the total payments made by each insurance
total medical visits for each insurance
and the the total individual patients per insurance.

Thanks in advance for any help

Grouping headers and footers are set to

[date from] Year / [INS1] / [Chart Number] / [date from] day / detail

Have the counter for the total patients per insurance in the [Chart Number]
footer and set to over group THIS IS WORKING FINE example 3 patients were
seen under medicare in 2009

Have the total patient visits counter in the [date from] day footer set to
over group but this only returns the total visits for the LAST patient seen.

Have tried to set to over all but it just totals the visit for the entire
report.
