Multiple copies of same document open

We had a user who could not "save" or "save as" the document she had been
working on. She was able to "select all" and make and save a new document.
When clicking on the "window" menu to move between the documents, I noticed
that her document was listed twice, once with (read only) and once without.
The folder she was working on is her personal folder on the network,
permissions limited to just her.

I have never seen this before, nor do I know how it can happen and I think
that may have something to do with why it would not save or allow a save as.

If one opens a document and then tries to open it again, Word will simply
take you to the window where the document is already opened. It does not
open a second copy of the document.

I have tried to duplicate this situation using "open as copy" or "open read
only" but window menu does not list docs as it did for this user. Her
window menu looked like this:

file name
file name (read-only)

Any ideas how this can occur?

It is quite possible (particularly when automating things) to end up with
multiple instances of Word running. While I can't give the exact steps to
reproduce the issue, I know that on a number of occasions where I have been
developing things, I have ended up with Word not responding in the way in
which I think is should and using Ctrl+Alt+Delete and going to the processes
tab, I have found multiple instances of WINWORD.EXE and if I am lucky and
kill the right ones, things return to normal. Otherwise, it's a case of
killing all of them and starting again.
