Here's the situation. I have a company with address, phone, etc.
information. This company has multiple sites. The company and sites have
multiple contact names and multiple phone numbers per name. Each site has
multiple pieces of equipment. Here's basically the way I would like the
report to look:

Another option would be to have the company name and address like above, but
list just one site with its multiple pieces of equipment and the relevant
contact information for that specific site. Like this:
