multiple tabs with leaders

I'm trying to set up a form that has several tabs and i need a line in
between each one, like 1. Date of Birth: _____Driver’s License #______ State
Issued:________
how do i set up tabs using leaders when i have more that 2 or 3 tabs.

However, the easiest way to obtain and to control the layout of a form is to
use a table. Add table borders where you want lines to appear (the column
width determines the line length). For example, type "Date of Birth:" in one
cell without borders and insert the related text form field in a cell with a
bottom border. By creating the form this way, the user will be able to fill
in the form electronically without breaking the design.
