Mutiple field search

Howdy folks,

I have a form, on this form I have multiple fields IE, FirstName,
LastName, Address, City, State, ZIP, CustID etc... there is a find button at
the bottom. What I am looking for is a search that will open a new form with
the results of the search. I have tried makeing a query that connects to the
form and another form that connects to the query but I am having mixed
results. Thank you for any assistance that you can offer.
