Need "Certificate of Completion" form for construction work

I am looking for a form "Certificate of Completion" that is filled out when
work has been completed and sent in to such as Insurance Companies for
payment.

Didn't the insurance company provide one?

No. When asked they stated that they do not have one to fax to me but need
one to finish with the payout on the claim.

Then they must not care what it actually looks like, so pretty much
anything you find will do. Look here for some.
